After Hours Emergencies

If you have an urgent matter after hours and need to speak to the physician on call, please call (801) 299-7629 and our answering service will page the physician.

If you experience a life-threatening emergency, PLEASE DIAL 9-1-1 IMMEDIATELY.

Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting your records to be sent to another provider we ask that you supply us with the name, address, and phone number.

Prescription Refills

We are happy to refill your medications as it is necessary. Please be sure to give us at least 24 hours notice on all refill requests.
